Older Adult Services

Geriatric Care Management

Internship will include training in the Electronic Health Record system, training, shadowing and co-practitioner experience in psycho-social assessments, care planning and care coordination.  Includes home visits.  May include cross training in macro areas, including program evaluation, focus group facilitation.  May also include experience with other programs/services.

Mind University

Internship will include training, shadowing and assistance in facilitating monthly workshops on brain healthy lifestyle, assistance in cognitive training classes and in outreach presentations in the community.  Internship may include cross training in macro areas, including program evaluation, focus group facilitation.  May also include experience with other programs/services.

Holocaust Survivor Services

Internship may include assistance with program development, assisting with Person Centered Trauma Informed Care trainings, outreach, and program evaluation.  The intern will also have the opportunity to learn about programmatic reports and learn about indemnification assistance for Holocaust survivors. They will also assist with Kosher Meals on Wheels in some capacity, which could include intakes and reviews with participants and/or delivery of meals. Internship may include cross training in macro areas, including program evaluation, focus group facilitation.  May also include experience with other programs/services.

Macro

Internship will include shadowing and learning about multi-agency collaborations and initiatives addressing community-wide needs and gaps in services. Internship will also include an opportunity to focus on a project identified and agreed upon by both intern and supervisor to do direct work, advocacy, community organizing, program evaluation, focus group facilitation, etc. Internship may also include cross training and experience with other programs/services.

Family Support Services

Internship will include training in the Electronic Health Record system, training, shadowing and co-practitioner experience in psycho-social assessments, treatment planning and service coordination.  Client appointments can include home visits and community based visits, as well as attending meetings in the community with collaborating agencies.  May include cross training in macro areas, including program evaluation, focus group facilitation.  May also include experience with other programs/services within JFS such as holiday programs and various programs in coordination with the volunteer department. MSW and BSW level students will have similar exposure in the program. Once trained, intern may be assigned a small number of FSS client cases.

Resource Center

Internship will include seeking out community resources and entering information into a resource database.  May include communication with callers via email or chat and training in the Electronic Health Record system.  Will include home visits for Kosher Meals on Wheels intakes and annual reviews.
