Geriatric Care Management
By partnering with the older adults we serve, as well as their families, our professional geriatric care managers work to identify needs with a thorough assessment that covers physical safety, nutrition, cognition, social wellness, medical history, and the legal and financial situation.
Once this information is gathered, a customized plan of care is developed and may include:
- Assistive technology (including personal emergency response systems)
- Counseling
- Friendly visitors
- Kosher Meals on Wheels
- Mind University (cognitive wellness)
- Transportation
- ElderCare Solutions of Michigan (fee-based 24/7 care management)
- Jewish Hospice & Chaplaincy
Geriatric care managers compassionately assist with sensitive issues such as:
- Hospital discharges
- Nursing home placement
- Driver cessation
- Family conflict resolution
- Guardianship
- Concerns of abuse or neglect
Jewish Family Service works closely with the Jewish Hospice & Chaplaincy Network to connect families to culturally- and religiously-sensitive resources and services for end-of-life issues.
