Home Care

Home care allows older adults to maintain control over their lives and enables them to live in the comfort of their own home, delaying or eliminating the often costly and less desirable institutionalized options. In addition, it can help ease the strain on family caregivers who are providing daily care for loved ones.

Bringing together area resources, a geriatric care manager designs a service plan based on the needs of each individual and their family. Some of these services may include:

  • Light housekeeping
  • Laundry
  • Grocery shopping
  • Errands
  • Personal care / bathing assistance
  • Meal preparation
  • Escorted transportation
  • Companionship
  • Respite for family caregivers
