Health Coaching Services

 

If we add up all the time we spend in doctors’ offices, it would likely come out to just a few days each year. And while it’s important to have medical professionals we can turn to for help, the reality is that we are in the best position to manage our own health. But there’s no reason to manage it all alone. We are here to help you take that step toward a healthier you.

Classes

No matter what kind of chronic condition you may have, we can empower you to take charge of your health. The Personal Action Toward Health (PATH) program was designed to provide the necessary skills and tools to help people with chronic health conditions (such as high blood pressure, diabetes, high cholesterol, arthritis, obesity, depression, anxiety and others) live a healthier life.  This fun, interactive six-week workshop teaches how to:

•    Create a personal plan for success
•    Develop problem solving skills
•    Improve communication with health care providers
•    Decrease stress
•    Make healthier food choices
•    Incorporate physical activity into your daily life

This program is open to adults, their family and friends, as well as caregivers.

Coaching

Maybe you’ve been struggling to drop ten pounds. Or add exercise into your daily life. Whatever your health goal may be, a coach can help identify the barriers to your success and brainstorm ways for you to be successful. Certified Health Coaches provide more in-depth and personalized guidance than what is typically available through standard healthcare providers. Research has found that health coaching is a promising strategy for helping people make behavior modifications that can prevent or manage many chronic conditions. Topics covered include, but are not limited to:

• Chronic disease management
• Self-care
• Nutrition
• Exercise
• Stress management
• Mindfulness and meditation
• Medication management
• Weight management
• Problem solving
• Other topics of interest

Coaching is available on a one-on-one basis and in groups (in our office or a community location), as well as in the workplace.

Workshops

Our team can present to your group or organization on a number of topics, such as brain health, stress management and normal aging, to name a few.
