Geriatric Case Management
As we age, daily living can present new challenges. For an older adult and their family members, it’s comforting to have someone support them as they work through those challenges.
With JFS’ Geriatric Case Management services, our goal is to ensure the greatest degree of independence for older adults, while maintaining safety and dignity.
Each of our Geriatric Case Managers partners with the older adult and family members to identify needs and develop a plan of care, customized to the older adult’s situation. Our professionally-trained Case Managers work with the family to carry out the care plan, applying their understanding of the intricacies of various federal and state programs, including:
- Medicare Part D
- Medicaid
- Food stamp assistance
- Senior living resources
- Health care, such as medical monitoring products and durable medical equipment.
In addition, the Geriatric Case Manager can assist with sensitive issues such as:
- Hospital discharges
- Nursing home placements
- Driver’s license removal
- Guardianship
- Adult protection
Most importantly, our JFS Geriatric Case Managers compassionately facilitate tough decision-making relating to transitions. All of this is done in a respectful and dignified manner during life’s stressful and potentially confusing times.
Families facing terminal illness are often unfamiliar with and confused by the diversity of health care and family support services available in the community. JFS partners with the Jewish Hospice & Chaplaincy Network to link families to resources and services on end-of-life issues that are culturally, religiously and linguistically sensitive.
