What is FCAN (Family Caregiver Access Network)?
The Family Caregiver Access Network (FCAN) is a national model, which uses a combination of outreach, assessment, and service coordination to promote service use by family caregivers of older adults.
Who is a family caregiver?
A family caregiver is an individual who provides unpaid care to family members and friends with physical, mental and/or emotional impairments.
Who is an appropriate FCAN referral?
Anyone who is currently a family caregiver for an older adult is a great referral.
What is the goal of FCAN?
The goal of FCAN is to provide access and services that can help family caregivers maintain their health, well-being, and ability to continue in the caregiving role through:
- Resource information and referral
- Respite care
- Support groups and counseling
- Case management
- Education and training
- Other support services

Family caregivers have chronic conditions at more than twice the rate of non family caregivers (AoA, 2004). Being a family caregiver can have negative effects on one’s health, work life and financial well being.
