Family Caregiver Access Network (FCAN)

JFS provides resources and referrals to assist people who are caring for older adults through our Family Care Access Network (FCAN). We also offer workshops to provide resources and support for family caregivers. Having information and support can help manage stress and also enable a caregiver to provide the best care possible to help their loved one age well.

PATH Program

Included in one of our informative workshops is the Personal Action Towards Health (PATH) program. The 6-week workshop teaches critical skills and tools to people who are living with ongoing health issues. If you are caring for someone with a chronic illness, this program is designed to help you as well as the person you are caring for. Chronic illness could include diabetes, heart disease, arthritis, osteoporosis and high blood pressure as well as others. Studies have shown that learning important coping techniques can help people manage their health, resulting in a positive change in their overall health and wellbeing.

For more information on our Chronic Disease Management Program or other caregiver workshops, visit www.mipath.org.

If you know anyone who is a family caregiver and would like support, they can contact the Resource Center at 248.592.2313 or contact us. All phone calls and correspondence are free of charge and are kept confidential.
