Who provides the
services?
-A Master's level professional evaluates the functional
capacity of the older adult
-A team of Jewish Family Service professionals will develop and initiate a plan of care
including referrals for additional services
-Professionals will continue to monitor and modify the plan
Where will the services be delivered?
Our professional staff will come to the older adult's residence whether it be a private
home, apartment, assisted living facility, nursing home, or elsewhere. Only by observing
the senior adult in his or her own surroundings, can an accurate assessment be
determined.
What are the fees?
Our rates include the initial assessment and ongoing service and monitoring. Our
professionals will discuss the fees with you by phone or in person.
How frequently will staff meet with clients?
The individual contacts will depend upon the needs of the client and significant others.
Service could consist of a one-time assessment or could mean weekly visits from social
workers.
Who might benefit from Care Connection?
-Those with busy families in town who need assistance in determining needs and
resources.
-Older adults with families out of town who need ongoing reassurance and evaluation.
-People who have guardians or conservators but who need case managers to assist in care
delivery and coordination.
-Seniors whose finances are handled by trust officers but who need assistance in
managing their personal and/or social functioning.

What is Care Management?
Care management is simply a formal name for the coordination of resources and services to
assure that older adults and others requiring assistance at home receive appropriate and
continuous support to meet their individual needs. A care manager identifies needs,
determines a plan for short or long-term care, arranges for delivery of services and
monitors those services to assure continuity of care.
What services are provided?
Assessment
Evaluation of older adult's psychological and social functioning, physical abilities,
living situation, and safety needs
Identification of support systems
Determining family, neighborhood, and community resources
Identification of immediate and ongoing needs
Evaluation of personal and social needs such as counseling, transportation, bill paying,
grocery shopping, meal preparation, and shopping
Resource identification
Determine what resources are available to meet the needs.
Development of customized Care plan
Assisting the family and others to assure an optimum quality of life. Providing service
coordination including referrals to outside agencies.
Follow-up and monitoring
Ongoing communication-both written and verbal-with family members and others to
collaborate and coordinate care, and evaluate needs.
